The Executive Director in the Ministry of Education, Innovation, Youth, Sport, Arts, and Culture, Mbumba Haitengela, has announced that the investigation found that the examination paper, which was written on 28 October, was not leaked.
The Ministry has also ruled out a rewrite of the 2026 NSSCO Grade 11 History Paper One, and an investigation confirmed that only some examination questions had been compromised before candidates sat for the national examination.
Haitengela in a statement confirmed that certain questions appearing in the national examination paper had been compromised through their inclusion in a regional mock examination written by candidates in the Erongo Region in August this year.
"The investigation attributed the compromise to the national examination paper setter, who breached established examination procedures by allowing some of the questions to appear in the regional mock examination."
The history examination was written on 28 October.
Haitengela explained that the remaining questions provide sufficient evidence of candidates' knowledge, understanding, and skills to support a fair assessment.
He also clarified that the incident did not affect the NSSCAS Grade 12 History examination.
Meanwhile, allegations of nepotism, favouritism, and irregularities in the appointment of examination personnel remain under investigation.
The allegations concern the appointment of examination setters, moderators, markers, team leaders, and chief markers.
Haitengela said appropriate action would be taken where wrongdoing is established and pledged to strengthen oversight of the appointment process.
The findings follow concerns over the security of national examination papers and the appointment of personnel responsible for setting and marking examinations.
